LEGAL WAIVER

1. By signing below, I acknowledge that participation in OU Israel day trips and/or any other OU activities may involve certain risks, which may include, but are not limited to, injury, accident, illness, damage to property or, in rare cases, serious harm or death.
2. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees (as defined below) or others, and assume full responsibility for my participation.
3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the organization immediately.
4. I represent that I am in good health and physically and mentally able to participate in OU Israel day trips and/or any other OU activities.
5. In the event that the I become injured or ill during any OU Israel day trips and/or any other OU activities, or if I must return home independently for reasons of health, accident, failure to conform to terms and conditions established as set forth above, or any other personal reason, I agree to accept full responsibility for and to pay for the cost of all medical care, transportation and other incidental expenses.
6. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release, indemnify, and hold harmless OU Israel, the Seymour J. Abrams Orthodox Union Jerusalem World Center, the Union of Orthodox Jewish Congregations of America (the "Orthodox Union"), and their respective officers, directors, trustees, affiliates, agents, employees, independent contractors, consultants, advisors, vendors, volunteers, licensees and assignees (collectively "releasees" and each a "releasee") from and against every claim, loss, damage and liability or responsibility, including, without limitation, all costs or expenses associated therewith, including but not limited to reasonable attorneys' fees and costs ("liabilities"), including claims for negligence on the part of any releasee or any other person or entity, claims for any delay, property damage, loss or theft, bodily injury, accident, illness, disease, death, disability, mental or emotional injury, and all other claims for damage, cost, loss or expense of any nature whatsoever, whether arising from the negligence of the releasees or otherwise, to the fullest extent permitted by law.
7. I have read and understood this release and assumption of risk agreement. I understand that by signing it, I am waiving certain legal rights, and I am doing so knowingly and voluntarily.
